About the Role
This fully remote position requires minimal travel (up to 10%) to client locations. We are looking for local candidates who currently reside or plan to move within a 90-minute drive of Houston, TX. Thank you for your understanding.
As a Medical Assistant - Healthguide, you will play a vital role in strengthening the connection between patients and their primary care physicians and medical practice staff. Your efforts will be instrumental in enhancing patients' health and well-being. You will engage with patients, assist them in navigating their care, and facilitate communication with their medical practice.
What You'll Be Doing:
Collaborating primarily from home, you will work closely with medical practice staff and our Case Management team to manage a targeted patient population.
Functioning as a key team member and extension of medical practices, you will operate independently while fostering collaboration.
Helping patients achieve better health outcomes by addressing their concerns, connecting them with medical practices, implementing effective care plans, closing care gaps, and navigating them to the right resources, including specialty care and community organizations.
Accurately documenting patients' medical histories and issues in the electronic health record (EHR).
Understanding the referral and prior authorization processes.
Building trusting professional relationships with assigned medical practices and within our organization.
Meeting assigned Key Performance Indicators (KPIs) to drive positive health outcomes and reduce medical expenses.
Utilizing effective communication methods (calls, texts, portals, emails) to engage high-risk patients.
Appropriately escalating medical issues to registered nurses, primary care physicians, or leadership.
Traveling occasionally to supported primary care physician offices.
Adhering to all security policies and procedures to protect patient information and proprietary data.
What You'll Need:
Certification or registration as a Medical Assistant.
Bilingual proficiency in English and Spanish is required.
A passion for patient coordination and serving individuals with complex medical and social needs.
Empathy and a willingness to learn motivational interviewing, critical listening, and behavioral change management skills to enhance patient engagement.
Exceptional communication skills with the ability to accurately document patient interviews in real time.
Experience with medical terminology and familiarity with the operations of primary care practices.
Knowledge of social determinants of health.
Certification as an Emergency Medical Technician is preferred.
Proficiency in using electronic health records (EHRs) and care management analytics databases.
Strong ability to prioritize tasks while remaining detail-oriented.
Familiarity with various EHR systems, such as EPIC, Athena, and Cerner.
Excellent written, verbal, and visual communication skills.
Proven ability to manage multiple tasks and ensure timely completion.
Ability to work independently as part of a patient care team under the supervision of licensed clinical professionals.
Experience successfully working in a remote environment.

Compensation:
The base pay for this role ranges from $20 to $23 per hour, paid bi-weekly. Final pay decisions consider various factors, including skills, experience, education, location, and certifications.
