Location: Fully Remote (with occasional travel to client locations, up to 10%). We are seeking candidates who reside or plan to reside within a 90-minute drive of Houston, TX.

Your Role:
As a Medical Assistant - Healthguide, you will play a crucial role in strengthening the connection between patients and their primary care teams. You will work to enhance patient health, navigate their care, and improve communication with their medical practice.
What You'll Do:
Remote Work & Collaboration: Primarily work remotely, collaborating with patients, their medical practice staff, and our Case Management team to manage a targeted patient population.
Patient Engagement: Assist patients in achieving better health outcomes by addressing their questions, connecting them with medical practices, and helping implement care plans. Facilitate referrals to specialty care and community-based organizations as needed.
Documentation: Accurately record patients' medical histories and issues in the electronic health record (EHR).
Referral Process: Understand and navigate the referral and prior authorization processes.
Relationship Building: Build and maintain professional relationships with assigned medical practices and within our organization.
Performance Metrics: Meet assigned Key Performance Indicators to ensure positive patient outcomes and reduce medical expenses.
Communication: Use various communication methods (calls, texts, portals, emails) to engage high-risk patients.
Issue Escalation: Appropriately escalate medical issues to the Health Guide Registered Nurse, Primary Care Physician, or leadership as needed.
Travel: Occasionally visit supported primary care physician offices.
Security: Follow security policies to protect all Protected Health Information (PHI) and intellectual properties.
What We're Looking For:
Certification: Certified or Registered Medical Assistant.
Bilingual Skills: Fluent in both English and Spanish.
Patient Coordination: Passion for coordinating care for patients with complex medical and social needs.
Empathy & Skills: Strong empathy, motivational interviewing skills, and the ability to engage patients effectively.
Documentation: Excellent conversation skills and experience with documenting patient interactions in real-time.
Medical Knowledge: Understanding of medical terminology, primary care functions, and social determinants of health.
Technical Skills: Experience with EHRs and care management analytics, and proficiency in various software programs.
Attention to Detail: Ability to balance priorities and track multiple tasks while maintaining accuracy.
Remote Work Experience: Demonstrated success in a remote work environment.

Compensation:
The hourly rate for this role ranges from $20 to $23, with final pay based on factors such as experience, education, and location.
